Public health nurses (PHN) provide services throughout Shelby County to appraise, protect and promote good health and the well being of our residents. This is accomplished through direct health care services, population based screenings, education, outreach and health promotion activities.
CMH (Complex Medical Help)
CMH links families of children with special health care needs to a network of quality providers and helps families obtain payment for the services their children need.
Communicable Disease Control
An essential public health service, communicable disease control personnel will report, investigate and work to prevent spread of infectious diseases in Shelby County. Shelby County health care providers must report all positive reportable diseases to the Health Department. An epidemiologist is on contract to perform early surveillance,provide statistical reports, and assist in disease investigations.

Health Fairs
The nursing and environmental health staff provide health promotion activities at health fairs for schools or businesses. Dermascans, Blood pressure, glucose, cholesterol screening are available at a nominal fee. Please fill out the Health Fair Request Form and mail to:

Bike Helmets
The Nursing staff also have bike helmets available at no charge. The helmets are fitted, along with education on proper use. See our Bike Helmet Information Sheet
Car Seat Safety: The Sidney-Shelby County Health Department car seat program aids parents and caregivers in making important decisions about how best to restrain their child passengers, providing assistance with properly using and installing car seats, and working to ensure that all families, regardless of the financial circumstances have access to the resources they need to help their children travel safely.
The Ohio Buckles Buckeye Program is a program that provides child safety seats and booster seats to eligible low-income families throughout Ohio. Shelby County residents who are eligible can receive a free car seat through the Sidney-Shelby County Health Department.

Cribs for Kids: The Sidney-Shelby County Health Department, in collaboration with Ohio Department of Health and Cribs for Kids is providing Infant Survival Kits to parents and caregivers in our community. The Infant Survival Kit includes a cribette, fitted sheet, sleep sack and educational safe sleep materials.
Help Me Grow’s Home Visiting Program provides first time expectant or new parent(s) with health and child development information. The goal of this program is to give expectant parents the information and support they need to be prepared for the birth of their child and provide ongoing support for families to understand and maximize this period of development to age 3.
Immunizations
The health department provides routine children and adult vaccinations to help prevent the spread of infectious diseases. A series of vaccines is usually required. This helps to build the immunity to the disease for which the vaccine is given.
All vaccine services are by appointment only (Monday through Friday) from 9:00-11:00 AM and from 12:30-2:30 PM. Evening appointments available until 6 p.m. every first and third Wednesday of each month. Insurance cards must be presented at each visit.
The health department provides international travel vaccines, adult, children, and infant immunizations. Click on the Centers for Disease Control website to view the childhood immunization schedule, the adult schedule and information on international travel vaccines. Charges exist for the adult and international travel vaccines along with an administration charge.
Newborn Home Visit
SSCHD partners with Help Me Grow and Family Connects Ohio to provide an in-home visit from a nurse who will offer support, guidance, and connections to additional resources—at no cost to you.
This free and voluntary visit is made by a Public Health Nurse trained in newborn issues and topics (SIDS, growth and development, breastfeeding, etc.). Nurses provide evidence-based personalized care by providing:
- Support for parents, family, and your baby
- Guidance and support on nutrition, sleep practices, and development
- Physical exam for mom and baby
- Connections to local resources: childcare, lactation, and mental health services

School Health
Public Health Nurses provide students with mandated vision and scoliosis screenings. Sidney City and County schools provide the student’s green health record with the school nurse completing and updating the record, and assist in kindergarten registration and assessment activities. Several schools contract for specific clinic hours staffed by the PHN. Health education in the classroom is completed by the PHN with topic such as puberty/body changes, communicable diseases, and health related topics. Health Fairs for the staff is available upon request.
