The Vital Statistics Registrar is responsible for recording all births and deaths in Shelby County. Records are available dating back to December 1908.

Beginning May 3, 2023 you may request and pay for Certified Copies of Vital Records by Clicking Here!

To make it easier for you to request birth and death certificates, a form has been provided on this page for you to print. Fill out the form, mail with required fee in the form of a money order (checks will not be accepted) made payable to the Sidney-Shelby County Health Department, along with a self addressed stamped envelope.
