• EVENING IMMUNIZATION CLINICS are being offered every first and third Wednesday of the month until 6 p.m. All vaccine services are by appointment only. Call (937) 498-7249 to schedule. Insurance cards must be presented at each visit. Flu vaccines and other immunizations available. We are now billing most private insurances.
